About the Disease-Specific Care Certification Program

To earn and maintain certification, a disease management program undergoes an extensive, on-site review by a team of Joint Commission professionals once every two years.  The program is evaluated against Joint Commission standards through an assessment of a program’s processes, the program’s ability to evaluate and improve care within its own organization, and interviews with patients and staff.

Because Joint Commission standards are regarded as the most rigorous in the industry, Joint Commission certified disease management programs have demonstrated their commitment to providing quality care to their patients. Joint Commission’s Gold Seal of Approval™ is a clear sign that certified programs have demonstrated compliance with the most stringent standards of performance.
